Please read these Terms carefully. By using midlabstaffing.com (the “Site”), you agree to these Terms of Use (“Terms”). If you do not agree, do not use the Site.
1) Who We Are
Mid Lab Staffing (“we,” “us,” “our”) is a staffing agency that connects healthcare employers with qualified laboratory professionals. The Site provides general information and forms to contact us. It does not provide medical advice.
2) Eligibility & Use
You must be at least 16 to use the Site. You agree to use the Site only for lawful purposes and in accordance with these Terms.
3) Not a Guarantee of Employment or Staffing
Submitting a form or resume does not guarantee job placement, interviews, or offers. For employers, submitting a request does not guarantee we will fill a role, timeframe, rate, or candidate availability. Any staffing engagement will be governed by a separate written agreement between you and Mid Lab Staffing.
4) User Submissions
You are responsible for information you submit (accuracy, legality, and that you have the right to share it). Do not upload patient PHI, social security numbers, bank info, or other sensitive identifiers unless specifically requested via a secure process during onboarding.
You grant us a non-exclusive, worldwide license to use your submitted information for staffing purposes, consistent with our Privacy Policy.
5) Prohibited Conduct
You agree not to:
Interfere with or disrupt the Site or servers
Attempt to gain unauthorized access to systems or data
Scrape, crawl, or harvest data without written permission
Upload malware, spam, or content that is unlawful, defamatory, or infringing
6) Intellectual Property
All Site content, logos, graphics, and trademarks are the property of Mid Lab Staffing or its licensors and protected by law. You may not reproduce, distribute, modify, or create derivative works without our written consent, except for personal, non-commercial viewing of the Site.
7) Third-Party Links & Services
The Site may link to third-party sites or services (e.g., analytics, form processors, job boards). We are not responsible for their content, policies, or practices. Review their terms and privacy policies.
8) Disclaimers
TO THE FULLEST EXTENT PERMITTED BY LAW, THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO ACCURACY, RELIABILITY, AVAILABILITY, NON-INFRINGEMENT, OR FITNESS FOR A PARTICULAR PURPOSE. YOUR USE OF THE SITE IS AT YOUR OWN RISK.
9) Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, MID LAB STAFFING AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, DATA, OR GOODWILL, ARISING FROM OR RELATED TO YOUR USE OF THE SITE, EVEN IF ADVISED OF THE POSSIBILITY. OUR TOTAL LIABILITY FOR ANY CLAIM RELATING TO THE SITE WILL NOT EXCEED $100.
10) Indemnification
You agree to defend, indemnify, and hold harmless Mid Lab Staffing and its personnel from any claims, damages, liabilities, and expenses (including reasonable attorneys’ fees) arising out of or related to your use of the Site or violation of these Terms.
11) Changes to the Site or Terms
We may update or discontinue the Site (in whole or part) without notice. We may modify these Terms from time to time; the Effective date reflects the latest version. Continued use after changes constitutes acceptance.
12) Governing Law; Venue
These Terms are governed by the laws of the State of Illinois, without regard to its conflict-of-laws rules. You agree to the exclusive jurisdiction of state and federal courts located in Cook County, Illinois for any dispute not subject to arbitration.
13) Dispute Resolution (Optional Arbitration)
At our election, any dispute relating to these Terms or the Site may be resolved by binding arbitration administered by a recognized provider, held in Chicago, Illinois, in English, on an individual (not class) basis. Either party may seek relief in small-claims court or injunctive relief in court for misuse of IP or the Site.
14) Electronic Communications & Consent
By submitting forms, you consent to receive communications (email, phone, SMS) related to staffing services. You can opt out of marketing at any time; operational messages about interviews or assignments may still be sent.
15) Severability; Waiver
If any provision is found unenforceable, the remaining provisions remain in effect. Our failure to enforce a right is not a waiver.
16) Entire Agreement
These Terms (plus the Privacy Policy) are the entire agreement regarding Site use. Any staffing engagement requires a separate, signed agreement.
